Internal menus - have different way of hooking to the game that allows them to alter game files to a higher extent,manipulating natives and changing and altering the game in ways that are a lot easier to sig and pick up on(almost limitless potential but more likely to be detect) External menus just modifying memory at certain addresses without injecting anything.(very hard to detect, but limited in function compared to internals) If you inject "code" into the game then you are working internally.
